Pathways To Wellness is a free local service that connects individuals and families in the Ely, Babbitt, and Tower area to resources to achieve their health and wellness goals.

Our skilled, trained professionals are located within the following organizations:

  • Essentia Healthy Ely Clinic

  • St. Anthony's Catholic Church

Who We Are

Pathways to Wellness is a local service that helps community members in the Ely, Babbitt, and Tower area connect to supports and services to improve their quality of life. Our Care Facilitators partner with individuals and families to improve health and wellbeing by identifying and addressing personal needs and goals, reducing barriers, and establishing support services. This is a free, person-centered, and confidential service.

What We Do

Our Care Facilitators assist you in developing a plan and working toward your goals. Some things care facilitators help with:

  • Safety

  • Housing

  • Education

  • Mental Health
  • Physical Health
  • Access to Food

  • Access to Basic Belongings

  • Recreation and Fitness
  • Transportation
  • Insurance
  • Accessing Medications
  • Employment
